You can find our eligibility requirements for prospective adoptive parents, the adoption application, and instructions on how to submit the $350 non-refundable application fee below. The completed application can be mailed to:

When submitting your application please include the following:
- Pet vaccination information (if applicable)
- Floor plans of your current residence with dimensions
- Photos of the outside play area to be used by the child
Eligible Adoptive Parents:
- Must be between the ages of 25 and 45
- Must have been married for a minimum of three years.
(there are some exceptions to this rule) - Must have a medical infertility issue
